Refer a patient

All referrals to the Cancer Center must be submitted on the official New Patient Referral form (PDF). Incomplete forms will be sent back to the referring physician.

• Forms must have at least 2 patient identifiers
• Please indicate the cancer type
• Required staging investigations must be complete prior to referring – please refer to page 2 of the referral form. If waiting for these investigations will cause harmful delay in access to care, please ensure they are scheduled and indicate the date on the referral.
• Referring physician contact information must be entered.
• All patients must be told of their diagnosis.

To ensure there are no unnecessary delays, please complete the form accurately before faxing to the Cancer Care Program.
